Treatment of severe meniere´s disease using gentamycine and betamethazone labyrinth perfusion
Abstract
For Meniere`s Disease (M.D.) patients, we are using Betahistin as standard therapy & additionally Urea or Acetazolamide for the attack periods, with good results. But in severe cases of invalidating M.D. causing extensive vertigo, we are using since 6 years labyrinth perfusion by a solution of Gentamycine and Betamethazon.
Methods: After insertion of a large ventilating tube, the patient is advised to drop 5 drops from an Otic Gentamycine + Betamethazon Solution (GARAMATR) 4-times daily, in a laying position with the treated ear on the top. Then he makes swallowing maneuvers and push inside the tragus, until he feels the bitter taste in his mouth. Then he lays down for 10 minutes each time.
This treatment continues until the patient remarks irritative vertigo or changes in hearing or tinitus. The perfusion is interrupted when a nystagmus of inverse direction occurs.
Results : 12 treated patients were free of vertigo for 2 till 5 years while in 5 cases we have repeated the procedure 2 or 3 times.
Conclusions : On the basis of these good results we strongly recommend this simple method in case of severe unilateral Meniere`s Disease.
